Open Schools, Covid-19, and Child and Teacher Morbidity in Sweden | NEJM

In mid-March 2020, many countries decided to close schools in an attempt to limit the spread of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the virus causing coronavirus disease 2019 (Covid-19).1,2 Sweden was one of the few countries that decided to keep preschools (generally caring for children 1 to 6 years of age) and schools (with children 7 to 16 years of age) open. Here, we present data from Sweden on Covid-19 among children 1 to 16 years of age and their teachers. In Sweden, Covid-19 was prevalent in the community during the spring of 2020.3 Social distancing was encouraged in Sweden, but wearing face masks was not.3
Data on severe Covid-19, as defined by intensive care unit (ICU) admission, were prospectively recorded in the nationwide Swedish intensive care registry. We followed all children who were admitted to an ICU between March 1 and June 30, 2020 (school ended around June 10) with laboratory-verified or clinically verified Covid-19, including patients who were admitted for multisystem inflammatory syndrome in children (MIS-C, which is likely to be related to Covid-19)4 according to the Swedish Pediatric Rheumatology Quality Register. The Stockholm Ethics Review Board approved the study. Informed consent was waived by the review board.

The number of deaths from any cause among the 1,951,905 children in Sweden (as of December 31, 2019) who were 1 to 16 years of age was 65 during the pre–Covid-19 period of November 2019 through February 2020 and 69 during 4 months of exposure to Covid-19 (March through June 2020) (see the Supplementary Appendix). From March through June 2020, a total of 15 children with Covid-19 (including those with MIS-C) were admitted to an ICU (0.77 per 100,000 children in this age group) (Table 1), 4 of whom were 1 to 6 years of age (0.54 per 100,000) and 11 of whom were 7 to 16 years of age (0.90 per 100,000). Four of the children had an underlying chronic coexisting condition (cancer in 2, chronic kidney disease in 1, and hematologic disease in 1). No child with Covid-19 died.
Data from the Public Health Agency of Sweden (published report5 and personal communication) showed that fewer than 10 preschool teachers and 20 schoolteachers in Sweden received intensive care for Covid-19 up until June 30, 2020 (20 per 103,596 schoolteachers, which is equal to 19 per 100,000). As compared with other occupations (excluding health care workers), this corresponded to sex- and age-adjusted relative risks of 1.10 (95% confidence interval [CI], 0.49 to 2.49) among preschool teachers and 0.43 (95% CI, 0.28 to 0.68) among schoolteachers (see the Supplementary Appendix).
The present study had some limitations. We lacked data on household transmission of Covid-19 from schoolchildren, and the 95% confidence intervals for our results are wide.
Despite Sweden’s having kept schools and preschools open, we found a low incidence of severe Covid-19 among schoolchildren and children of preschool age during the SARS-CoV-2 pandemic. Among the 1.95 million children who were 1 to 16 years of age, 15 children had Covid-19, MIS-C, or both conditions and were admitted to an ICU, which is equal to 1 child in 130,000.

World Economic Forum Founder and Executive Chairman Klaus Schwab has proposed using the overreaction to coronavirus to launch a worldwide “Great Reset.” This Great Reset is about expanding government power and suppressing liberty worldwide.
Schwab envisions an authoritarian system where big business acts as a partner with government. Big business would exercise its government-granted monopoly powers to maximize value for “stakeholders,” instead of shareholders. Stakeholders include the government, international organizations, the business itself, and “civil society.”
Of course, government bureaucrats and politicians, together with powerful special interests, will decide who are, and are not, stakeholders, what is in stakeholders’ interest, and what steps corporations must take to maximize stakeholder value. People’s own wishes are not the priority.
The Great Reset will dramatically expand the surveillance state via real-time tracking. It will also mandate that people receive digital certificates in order to travel and even technology implanted in their bodies to monitor them.

We are being told that our liberties must be suspended in order to keep hospitals from reaching apocalyptic levels. But what if those levels are just above normal and not anywhere near apocalyptic levels? And what if these lockdown measures do nothing to keep the levels down anyway?
Well, if there is anywhere we can cross-check this hypothesis, it would be in Florida, where there is no lockdown or mask mandate. In fact, people are flocking there from out of state to enjoy vacations and host conferences and even to live. Naturally, we’d expect hospital levels to be bursting at the seams if they rise and fall with lockdowns and masks, right?
Well, actually, you can barely see an increase in the hospitalization level in the Sunshine State from previous years, and the current level appears to be on par with the 2018 flu season, which was more of a pandemic flu than other flus in recent years. And in 2018, we did nothing as a nation to suspend liberties.
There is much debate over how to count a COVID hospitalization given the rampant and unprecedented testing of people relative to past flus. But one easy way to observe an apples-to-apples comparison to past flu seasons is to compare the overall average daily census of hospitalizations now to previous years and adjust those numbers per capita to existing population. In other words, if all of the COVID patients are legitimately there because of COVID, we would see an enormous excess in the total number of people in the hospital at any given moment for any ailment. Florida is simply not seeing a gigantic increase.
Here is how the math works: HHS tracks total daily hospital levels in all the states dating back to Jan. 1, 2020. If you take the average daily total hospitalization levels in Florida for the fourth quarter of 2020, you will find an average (some days are more, some are less) of 43,150.
Naturally, I wondered what the levels were in previous years, because the Florida Agency for Health Care Administration publishes quarterly data of hospital censuses for several recent years. I started with the first quarter of 2018, which included the harshest flu season we had in a decade. If you average the total hospital census over the 90 days from Jan. 1 to March 31, it works out to 41,094 people in the hospital on an average day. Adjusting for the population at the time, that would be 1,972 hospitalizations per 1 million people. That is compared to 1,998 per 1 million for this past quarter of 2020 with COVID as the predominant illness.
As you can see, although the hospital numbers for the fourth quarter of 2020 were about 6%-8% higher than in the fourth quarter of the previous two years, it was barely higher than the first quarters of every year. The reason it is fair to compare to the worst months of previous years is because it has become clear that the flu is gone for this year and that COVID-19 is this year’s version of the flu. Thus, with flu cases down 98.8%, it is reasonable to assume that the January census will not grow as it typically does during peak flu season.
